Responsibilities
- Aid in outreach and engagement of community members in the E Robert Moore and Courtlandt PACT-RAD development
- Directly serve clients through intake, engagement, assessment, service plan development, referral, advocacy, counseling, exit summaries, and follow-up
- Manage a case load and document client progress towards service plan goals
- Maintain knowledge of program resources available both on- and off-site
- Coordinate client services with various providers including social service, mental health, medical, employment, educational, childcare, and others
- Complete program reports on a monthly basis or more frequently as required
- Respond to inquiries about clients and act as a program contact person
- Collaborate and support clients in securing benefits, particularly related to eviction prevention
- Report to and meet with supervisor monthly
- Conduct home visits and other field work as needed
- Facilitate monthly activities that meet the needs of the PACT-RAD community
- Assist with community events and serve as a liaison with community vendors
Qualifications
- Bachelor’s degree with case management experience or High School Diploma with 2 years of case management experience
- Experience using Access HRA and securing HRA entitlements and benefits preferred
- Mental health background preferred
- Bilingual preferred
- Crisis prevention and mental health first aid training is required within the first 6 months
- Strong oral and written communication, time management, and organizational skills are necessary
- Proficiency in Microsoft Office suite and other standard business technology is required
Physical Requirements
- Ability to use a computer for prolonged periods
- Occasional lifting and/or moving up to 10 pounds
- Standing, walking, or sitting for long periods of time
- Climbing five flights of stairs if required for home visits and fieldwork
- Bending and retrieving objects/documents
- Traveling in the boroughs of New York City and its adjacent counties via public transportation
- Traveling for a minimum of 1 mile within a reasonable amount of time and in all types of weather, including inclement weather, if required for home visits and fieldwork
