Case Manager - 40 hours - Days - Southfield

Responsibilities

• Collaborative practices of identification, assessment, planning, facilitation, care coordination, evaluation and advocacy for options and services to meet an individual's and family's health care needs.
• Provide effective discharge planning services that support a safe transition to the next level of care and address the needs of patients who have experienced a critical event or diagnosis.
• Work collaboratively with interdisciplinary teams to ensure comprehensive care plans are developed and implemented.
• Assess patient needs and develop care plans to optimize health outcomes along the care continuum.
• Facilitate access to community resources to meet the needs of diverse populations.
• Advocate for patients and their families, ensuring their voices are heard and their needs are met.
• Ensure compliance with regulatory requirements such as Medicare and Medicaid regulations and case management principles.
• Stay updated on preventive service guidelines, clinical practice guidelines, and behavior change theory.
• Manage multiple complex patients' needs simultaneously, demonstrating strong organizational and planning skills.
• Show compassion and provide emotional support to patients and their families.
