Responsibilities

• Serve as care coordinators for patients, caregivers, and families
• Conduct concurrent and retrospective review to ensure appropriate utilization, assessment, prevention of denials, and coordination of discharge options
• Follow specific criteria sets to ensure regulatory compliance
• Coordinate safe discharge plans between the health care delivery team, patients, families, insurers, and continuing care agencies

Qualifications

• Graduate of an accredited school of nursing (BSN preferred)
• 2-3 years of Case Management experience preferred
• Current Massachusetts RN licensure
• Certification in Case Management preferred
• Influenza vaccination required (as a condition of employment)

Pay

The pay range for this position is $79,691.61 USD – $176,178.38 USD annual base salary. This position is covered by a collective bargaining agreement; the pay range is established and governed by the terms of the applicable collective bargaining agreement, and employees hired will receive compensation consistent with those terms.
