Manager - Integrated Case Management - Full Time - Days - Southfield

About the Role

The Integrated Case Management Manager for Population Health is an interdependent member of the patient-centered care team responsible for the collaborative practice of assessment, planning, facilitation, care coordination, evaluation, and advocacy. This role addresses the comprehensive health care needs of individuals and families through communication and available resources to promote patient safety, quality of care, and cost-effective outcomes.

This position focuses on patients who have experienced a critical event or diagnosis requiring complex management strategies and extensive resource use to optimize health outcomes along the care continuum. Services are provided to patients in ambulatory, inpatient, or health plan settings.

Responsibilities

• Conduct assessment, planning, facilitation, and care coordination for patients and families.
• Advocate for options and services to meet comprehensive health care needs.
• Address complex patient needs resulting from critical events or diagnoses.
• Utilize resources to optimize health outcomes across the care continuum.
• Provide services in ambulatory, inpatient, or health plan settings.
• Apply knowledge of preventive service guidelines, clinical practice guidelines, and behavior change theory.
• Navigate Medicare and Medicaid regulations and case management principles.
• Understand medical ethics and legal implications related to case management.
• Assess social determinants of health and their impact on patient wellbeing.
• Facilitate community resources to meet the needs of diverse populations.
• Manage multiple complex patient needs simultaneously with strong organizational and planning skills.
• Advocate compassionately for patients and their families.

Requirements

• Bachelor’s degree in nursing or a related professional field (e.g., social work, counseling, health education) or a Master’s Degree of Social Work.
• Minimum three (3) years of clinical experience.
• Excellent verbal communication and written documentation skills.
• Strong customer service and interpersonal skills, including the ability to interact with internal and external customers and all levels of the organization.
• Strong problem-solving, analytical, and decision-making skills.
• Proficiency in computer skills and knowledge.
• Experience in discharge planning, home health care, rehabilitative medicine, community health, or managed care preferred.

Qualifications

• Registered Nurse (RN) or Licensed Master Social Worker (LMSW) with a valid, unrestricted State of Michigan license.
• Certification in Case Management (CCM) by the Commission for Case Management Certification (CCMC) preferred. For new hires, certification must be obtained within 3 years of hire.

Oncology-Specific Requirements

• Three (3) or more years of oncology experience preferred.
• Certification in oncology social work (OSW) preferred if the candidate is an LMSW. For new hires, certification must be obtained within 3 years of hire.
