RN-Case Manager - Part Time - Days - Southfield

Responsibilities

• Creates plan for care across the continuum, integrating patient/family preferences and values.
• Makes sure patients get the right level of care through assessments, ongoing evaluations, and/or patient records.
• Works to find resources and remove any roadblocks that might stop good care.
• Keeps talking to your boss and other team members to make sure everything works well and gets better over time.
• Helps make sure doctors have all the information they need in their patient files.
• Suggests what kind of care patients might need after they leave the hospital.

Requirements

• Diploma from an accredited school/college of nursing OR professional licensure at time of hire.
• Bachelor of Science in Nursing (BSN) preferred.
• Acute care case management experience preferred.
• Experience with ECIN/Allscripts/Careport, Google, Outlook, and Microsoft products preferred.

Qualifications

• Registered Nurse credentialed from the Michigan Board of Nursing obtained prior to hire date or job transfer date required.
• BLS Provider preferred.
• Certified Case Manager from the Commission for Case Manager Certification (CCMC) or the American Case Management Association (ACMA) preferred.
