RN Case Manager - Part-Time

We are seeking a registered nurse with a minimum of three years of clinical experience, including two years in Utilization Review, Utilization Management, or Case Management. Key Responsibilities Include Establishing safe, individualized discharge plans to promote quality healthcare outcomes.Providing proficient and timely utilization management services to optimize reimbursement for patient visits.Collaborating proactively with patients, families, and healthcare teams to coordinate care effectively. Qualifications And Skills Graduate of an accredited nursing school with current RN license in the state.Strong knowledge of social and physical factors affecting discharge and familiarity with community resources.Proficiency in computer applications such as word processing, spreadsheets, and data management.Ability to provide age-appropriate care tailored to patient needs.
