Pay Range: $32.88 - $40.45 Hourly
About the Role

This position provides case management services for children and adults with developmental disabilities. The State Office of Seniors and People with Disabilities contracts with Multnomah County to deliver these services. Case management helps eligible individuals access, coordinate, and ensure delivery of services and supports under their individualized plan, including medical, social, educational, and other needed services.
Responsibilities
- Manage a caseload of clients found eligible for DDSD case management services.
- Monitor services and supports via home visits, service provider visits, and phone calls to ensure clients receive appropriate financial, medical, training, educational, transportation, and other identified services.
- Develop and complete Individual Service Plans in client/family homes and provider settings.
- Develop, facilitate, and coordinate resources to provide consumers and families with needed services.
- Prepare and submit routine and special reports.
- Assess and review level of functioning and supports needed by completing and updating TXIX Waiver Forms annually.
- Inform the state when a client is out of a funded facility for any reason.
- Maintain case records and document actions in accordance with federal, state, and local policy; adjust payments and program benefits as needed.
- Counsel and advise consumers and families on placement, care, service plans, personal and emotional difficulties, financial planning, and interactional dynamics.
- Liaison with educational systems to support access to educational services meeting developmental disabilities needs.
- Report possible protective services issues and provide initial information to assist investigations.
- Use Tableau data and reports to analyze overdue tasks and self-direct monitoring of ODDS-required service priority areas, including ISP, Financial, Medical, Behavioral, and/or Employment services.
- Act as a liaison between residential community providers, licensing program specialists, service coordinators, supervisors, ACHP, and IDDSD management on trends, data, and issues related to health, safety, and regulations.

- Ability to interpret and apply complex federal, state, and local laws, rules, regulations, policies, and procedures.
- Skill in researching, analyzing data, making recommendations, preparing reports, and presenting information.
- Knowledge of case management methods, principles, techniques, and resources (including assessment, advocacy, and risk intervention).
- Knowledge of available programs, public assistance providers, Medicaid, service agencies, protective services, referral centers, community resources, and crisis/emergency resources.
- Knowledge of conflict resolution techniques, de-escalation strategies, and ability to facilitate large team meetings to achieve consensus.
- Promote a culture of respect, inclusiveness, and appreciation of diverse perspectives, backgrounds, and values.
- Valid driver’s license and ability to pass a criminal background check.
Qualifications

- Minimum Qualifications: Knowledge of the public service system for developmental disabilities services in Oregon and at least one of the following:
- A bachelor’s degree in behavioral science, social science, or a closely related field.
- A bachelor’s degree in any field and one year of human services-related experience (e.g., assisting people with economic disadvantages, disabilities, abuse, neglect, substance abuse, aging, health, cultural competencies, or housing).
- An associate’s degree in behavioral science, social science, or a closely related field and two years of human services-related experience.
- Three years of human services-related experience.
- Preferred Qualifications: Experience with:
- Relevant laws and resources.
- Conflict resolution and de-escalation techniques.
- Cultural sensitivity in case management.

This position is designated as hybrid telework, requiring both remote work and in-person attendance at the onsite location: Five Oak, 209 SW 4th Ave, Portland, Oregon 97209. All employees must reside in Oregon or Washington.
