CASE MANAGEMENT DUTIES
- Maintain a caseload of active clients
- Collaborate with multidisciplinary/multiagency staff to monitor clients’ progress
- Create and implement individualized plans or strategies to promote identified goals
- Advocate for individual clients around entitlements and services with follow-up
- Address procedural or programmatic barriers for households accessing services
- Stay updated on policy trends and changes in relevant areas
- Document and update client records in accordance with agency policy
- Participate in case conferences and team meetings
- Attend housing court to assist clients with eviction cases
- Update and maintain a comprehensive database on participants and services provided
- Review cases with the Casa Project Team weekly
- Meet clients in the field as needed

PAY RANGE
$50,000 to $60,000/year, based on work experience, life experience, qualifications, and education level
