Casa Project Advocate (Housing Coordinator)

CASE MANAGEMENT DUTIES

  • Maintain a caseload of active clients
  • Collaborate with multidisciplinary/multiagency staff to monitor clients’ progress
  • Create and implement individualized plans or strategies to promote identified goals
  • Advocate for individual clients around entitlements and services with follow-up
  • Address procedural or programmatic barriers for households accessing services
  • Stay updated on policy trends and changes in relevant areas
  • Document and update client records in accordance with agency policy
  • Participate in case conferences and team meetings
  • Attend housing court to assist clients with eviction cases
  • Update and maintain a comprehensive database on participants and services provided
  • Review cases with the Casa Project Team weekly
  • Meet clients in the field as needed

PAY RANGE

$50,000 to $60,000/year, based on work experience, life experience, qualifications, and education level
