Care Transition Liaison - Home Health

Role Overview: The Care Transition Liaison facilitates smooth patient transitions to home health or hospice services by acting as a resource for referred patients, their families, and healthcare providers. Key Responsibilities Coordinate patient referrals, ensuring adherence to the Right of Choice processCollaborate with physicians and case managers to identify patient needs and obtain approval for admissionGather and send necessary medical records to the agencyFacilitate start-of-care procedures between hospital/physician and home health agencyAssist in completing order sets and processing verbal orders as permitted by licensureSupport physician office referrals, including medication refill requests when applicableCommunicate concerns or complaints from physician offices or hospitals to leadershipServe as a resource for service options available from home healthDocument activities and productivity using designated systems Qualifications Valid driver's license, vehicle insurance, and reliable transportationPreferred: LPN/LVN or licensed medical professional with experience in hospital, physician’s office, home health, or hospice settings Skills & Attributes: Strong coordination, communication, and documentation skills to ensure effective patient care transitions and collaboration with healthcare teams.
