About the role
The Care Navigator provides ongoing proactive patient services through regular communication and personalized care plans for patients participating in the Chronic Care Management Program. They assess patient health status, address concerns, and facilitate access to community resources to improve overall well-being.
Responsibilities
- Provide ongoing proactive services through monthly calls
- Review the patient's current health status and appropriately escalate concerns to the physician via the escalation protocols
- Provide education related to medical conditions and factors impacting patient’s health
- Set health-related goals based on medical conditions & patient’s priorities
- Identify and assist in closing clinical and non-clinical care gaps
- Empower patients to make sustainable health behavior changes to improve their well-being
- Continually provide motivation, encouragement, and support
- Communicate with patient’s medical providers regarding important items such as new or worsening symptoms or medication access and adherence (per protocols)
- Share community resource options to help patients access the necessary resources at the right time (e.g., transportation, food)
- Create updates and maintain care plans using SelectPatient Management's proprietary software
- Achieve monthly call goals based on Supervisor Care Navigator advisory on assigned patient caseload
Requirements
- Certified or Registered Medical Assistant required
- 2 or more years of experience in healthcare or a related field (3-5 years preferred)
- Family practice, internal medicine, and geriatrics office experience preferred

- EFFECTIVELY and APPROPRIATELY communicate with doctors and healthcare professionals ensuring clarity, professionalism, and accurate information exchange to support patient care and coordination
- Strong communication and interpersonal skills (written and verbal)
- Proficient computer skills including Electronic Health Record, Microsoft programs, and Google Suite
- Work well independently in a fast-paced results-driven remote work environment

Pay
$18/hour base pay plus monthly bonus opportunities based on productivity
