CARE NAVIGATOR
Responsibilities
The Outpatient Navigator works under the guidance of the Clinical Director. The Outpatient Navigator assists with the overall internal growth of the outpatient programs. The Outpatient Navigator identifies, educates, and assists in facilitating individuals from the inpatient unit to the outpatient programs. The Outpatient Navigator also works to improve the overall patient flow and access to outpatient care.
Qualifications
- High School Diploma/GED
- Bachelor's Degree or Associate's Degree with at least 6-months of behavioral health experience preferred
- Must be at least 21 years of age
- Bachelor's degree required, Master's degree preferred
- Minimum of 2 years of experience in case management related work
- Valid Fingerprint Clearance Card
- Completion of BLS and First Aid Training within 30 days of employment
- Completion of Handle With Care Training within 30 days of employment
