About the role
The Care Navigator role is responsible for providing comprehensive patient care coordination services. This includes managing patient interactions, ensuring timely medication management, and facilitating seamless transitions between healthcare providers.
Responsibilities
- Manage patient interactions and ensure timely communication
- Coordinate medication management and adherence
- Facilitate smooth transitions between healthcare providers
- Provide education and support to patients and families
- Adhere to HIPAA guidelines and maintain patient confidentiality
Requirements
- Bachelor’s degree in Healthcare Administration, Nursing, or related field
- Minimum 2 years of experience in healthcare coordination or related field
- Strong organizational and communication skills
- Proficiency in Microsoft Office Suite
Qualifications
- Current CPR certification
- Knowledge of electronic health records systems
- Experience with telehealth platforms
