Care Manager RN - Managed Care Support
About the role
Are you a passionate Registered Nurse ready to make a real difference in the lives of patients managing chronic illness? As a Care Manager RN, you'll be at the heart of transforming patient care — serving as the central link for an identified patient population and collaborating with multiple PCPs to deliver coordinated, whole-person care.
Responsibilities
- Bring your expertise in assessment, education, and community resource coordination to empower patients in building self-efficacy and management skills, helping them achieve optimum functional health status and an improved quality of life.
- Partner with patients, families, and care teams to deliver high quality, evidence-based, cost-effective, and patient-focused outcomes.
- Work with a meaningful degree of autonomy within established nursing standards and the nursing code of ethics, including Chapter 18.88 RCW, to lead and grow while performing a full range of ambulatory care RN duties.
Requirements
- Coursework/Training: Registered Nurse
- Upon hire: Washington Registered Nurse License
- 2 years work experience in an RN role
Preferred Qualifications
- Experience managing complex patients in an ambulatory setting
- Medical Home Model experience

Pay
The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
