Care Manager RN - Managed Care Support

About the role

Seeking a dedicated Registered Nurse (RN) Care Manager to lead patient-centered care for individuals managing chronic illnesses. In this role, you will serve as the primary link within a patient population, collaborating with primary care providers to deliver coordinated, evidence-based, and cost-effective care.

Responsibilities

• Conduct assessments
• Provide patient education
• Coordinate community resources to empower patients in self-management
• Aim to improve functional health and quality of life
• Work with autonomy and within nursing standards and ethics
• Partner with patients, families, and care teams to achieve optimal outcomes
• Perform a full spectrum of ambulatory care RN duties

Qualifications

• Licensed Registered Nurse in Washington
• At least 2 years of RN experience

Skills

• Preferred experience managing complex patients in ambulatory settings
• Experience with Medical Home Model
• Strong assessment, education, and community resource coordination skills
• Ability to work collaboratively within care teams
• Autonomy within established nursing standards and ethics
