RN - Care Manager
About the role
The Inpatient Registered Nurse (RN) Care Manager provides professional, comprehensive, patient centric care management services for at risk patients in an acute care environment. Accountabilities include assessment and planning, coordination of care, resource utilization management and/or review, discharge planning, documentation of interventions, regulatory compliance and patient advocacy. The goal of the Inpatient Care Management RN is to ensure the use of appropriate healthcare resources throughout the continuum, so that the care provided is the right care, at the right time, in the right setting.
Responsibilities
- Assessment and Planning: Conduct thorough assessments of patients' needs and develop personalized care plans.
- Care Coordination: Coordinate care among various healthcare providers, ensuring seamless transitions between inpatient and outpatient settings.
- Resource Utilization Management: Review and optimize the use of healthcare resources to enhance patient outcomes.
- Discharge Planning: Develop and implement discharge plans to facilitate safe transitions from the hospital to home or another level of care.
- Documentation: Maintain accurate and timely documentation of interventions, ensuring regulatory compliance.
- Patient Advocacy: Advocate for patients' rights and preferences, ensuring their voices are heard and respected.
Requirements
- Associate's Degree in Nursing degree/diploma upon hire.
- Washington Registered Nurse License.
- 2 years Acute Care experience in a Medical Surgical or Inpatient setting.
- 1 year experience in care management or utilization review in any setting or upon hire successful completion of TIPS program or Case Management Orientation Program.
Qualifications
- Bachelor's Degree in Nursing or higher within 3 years of hire.
- National Certification in area of specialty.

- Excellent communication and interpersonal skills.
- Ability to manage multiple tasks and prioritize effectively.
- Proficiency in electronic health record systems.

Pay
The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
Schedule
This position is available on a per-diem basis.
