Care Manager I Social Work (MSW) - Per Diem : Irvine Care Management

Primary Duties and Responsibilities

  • Coordinate patients’ needs through the continuum of care, including pre-admission through post-discharge plans.
  • Collaborate with physicians, nurses, clinical staff, and community agencies to identify and arrange for appropriate care.
  • Review clinician assessments and patients’ financial, family, and psychosocial support to develop comprehensive care and/or discharge plans.
  • May focus on specific aspects such as discharge planning, utilization review, and psychosocial support.
  • Review records to assess appropriate admission status, level of care, payer source, and UR contracts to validate billing.
  • Provide psychodynamic intervention and crisis counseling to support patients and families.
  • Educate patients and families on their healthcare options and connect them with resources.
  • Document pertinent patient issues, contacts, and plans on medical records.
  • Act as a mandated reporter for elder, child, and spousal abuse.
  • Support the high-risk, homeless, and mental health population in the Community Care role.
  • Perform other duties as assigned.

Qualifications

  • Care Manager (MSW): Master’s degree in Social Work (MSW) required. One year of MSW experience in an acute healthcare setting preferred.
  • Community Health: MSW: Master’s degree in Social Work (MSW) required. One year of MSW experience in community case management preferred.
  • Understands working with the low-income and under-resourced community.
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
  • Bilingual or multilingual preferred.
