The Care Coordinator will serve patients across various age groups and developmental stages, including neonates, pediatrics, adolescents, adults, and geriatrics.
Responsibilities
- Identify patients and provide clinical social work and behavioral health services
- Conduct discharge planning evaluations and coordinate care
- Assess patient needs and develop psychosocial and care coordination/discharge plans
- Collaborate with interdisciplinary teams and patients/families to implement care plans
- Document social work discharge planning activities in the patient’s chart according to departmental, hospital, CMS, QIO, DHHS, and JC standards
- Prepare monthly statistical and other assigned reports
- Develop and maintain community relations
- Participate in professional staff development and hospital/community activities
Requirements
- Master of Social Work (MSW) from an accredited graduate school of social work
- NJ Social Work License: Licensed Clinical Social Worker (LCSW) or Licensed Social Worker (LSW)
- NPI and Medical Non-Billing Provider Number required; must be obtained within the first 30 days of employment
- Previous hospital experience preferred
- Experience in medical social work, perinatal pediatrics, behavioral health, or case management preferred
- Some weekend and holiday coverage is included
- EARN-I and PAS certified within 3 months of hire when applicable to the assignment
Pay
Salary Range: $40.00 - $40.00 USD
Competitive base rates are determined by factors such as job-related work experience, internal equity, and industry-specific market data. Some positions may also be eligible for clinical certification pay and shift differentials. The salary range listed for exempt positions reflects full-time compensation and will be prorated based on employment status.
