Responsibilities
The Inpatient LCSW Care Manager:
- Assesses and plans comprehensive care for at-risk patients in an acute care environment.
- Coordinates care and develops discharge plans.
- Documents interventions and ensures regulatory compliance.
- Promotes patient advocacy and addresses social, financial, cognitive/behavioral, or legal barriers to care access.
The Inpatient MSW Care Manager:

Requirements
Inpatient LCSW Care Manager:
- Master's Degree in Social Work from an Accredited Council on Social Work Education program.
- Washington Clinical Independent Social Worker License upon hire.
- 2 years SW experience in any healthcare setting.
- 1 year experience in acute care setting or successful completion of TIPs program or Case Management Orientation Program.
Inpatient MSW Care Manager:
- Master's Degree in Social Work from an accredited CSWE program.
- Washington Clinical Independent Social Worker Associate License upon hire.
- 1 year SW experience in any healthcare setting.
- 1 year experience in acute care setting or successful completion of TIPs program or Case Management Orientation Program.
Qualifications
Inpatient LCSW Care Manager:
- Basic understanding of acute medical surgical healthcare terminology.
- Prior case management experience.
Inpatient MSW Care Manager:
- ACM/CCM certification.
- Basic understanding of acute medical surgical healthcare terminology.
- Prior case management experience.
Skills
Both roles require:
- Excellent communication skills.
- Strong organizational and problem-solving abilities.
- Ability to work independently and as part of a team.
