Care Manager MSW
About the role
Under supervision of Care Management Manager, works as a catalyst to promote coordination of care, continuity of care, achievement of consistent and/or expected psychosocial outcomes, patient advocacy and efficient resource management. Coordinates the care for complex designated patients from pre-admission to immediate post-discharge.

Requirements
- Master's Degree in Social Work or related field
- Washington Clinical Independent Social Worker Associate License
- 3 years of Social services experience with a health care related organization; experience should include a minimum of 2 years in acute care/ care coordination.
Qualifications
- Upon hire: Accredited Case Manager (ACM) Or
- Upon hire: Certified Case Manager (CCM)

Pay
The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
