About the role
The Care Manager MSW at Providence St. Joseph Hospital Orange is a Per Diem position working 8-hour, Day shifts.
Responsibilities
- Assessment and planning of patient care needs
- Coordination of care and discharge planning
- Documentation of interventions and regulatory compliance
- Patient advocacy and support
Requirements
- Masters Degree in Social Work from an accredited CSWE program
- One year of Social Work experience in any healthcare setting
- One year of experience in acute care setting or successful completion of TIPs program or Case Management Orientation Program
Qualifications
- Basic understanding of acute medical surgical healthcare terminology
- Prior case management experience preferred
Skills
- Excellent communication and interpersonal skills
- Strong organizational and problem-solving abilities
- Ability to work independently and as part of a team

Pay
The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. The base pay range is $38.56 - $59.88.
Schedule
This position works Per Diem and will work 8-hour, Day shifts.
