Care Manager MSW Per Diem Day

We are seeking a Care Manager MSW to provide comprehensive, patient-centered care management services in an acute care setting. Responsibilities include assessment, care planning, coordination, discharge planning, documentation, regulatory compliance, and patient advocacy. The role aims to ensure continuity of care for vulnerable patients by identifying and addressing social, financial, cognitive, behavioral, or legal barriers to access. Qualifications include a Master's Degree in Social Work from an accredited program, at least one year of healthcare social work experience, and familiarity with acute care or successful completion of relevant training programs. Preferred skills include understanding of medical-surgical terminology and prior case management experience.
