Care Manager - Full-Time Day

Overview The Care Manager MSW plays a vital role in providing comprehensive, patient-centered care management services within an inpatient hospital setting. Responsibilities include conducting assessments, developing care plans, coordinating discharge planning, documenting interventions, ensuring regulatory compliance, and advocating for patients. The goal is to facilitate continuity of care for vulnerable patients by addressing social, financial, behavioral, or legal barriers to access. Qualifications Master's degree in social work from an accredited CSWE program.At least one year of social work experience in healthcare, including acute care or completion of relevant training programs.Preferred: Basic understanding of medical-surgical healthcare terminology and prior case management experience.
