Care Manager RN Full-time Day

About the role

The RN Care Manager is responsible for carrying out effective case management functions for identified patients by planning, coordinating, negotiating, procuring and managing the care and needs of complex patients and their families throughout the continuum of care. Communicates daily with medical staff, hospital personnel and outside resources to ensure appropriate utilization of resources according to clinical and financial standards. Monitors the medical record for variances to clinical and financial standards and intervenes to achieve the optimum outcome for the patient and the hospital.

Responsibilities

• Plan, coordinate, negotiate, and procure care for complex patients and their families.
• Manage care needs throughout the continuum of care.
• Communicate daily with medical staff, hospital personnel, and outside resources to ensure appropriate resource utilization.
• Monitor medical records for variances to clinical and financial standards and intervene to achieve optimal outcomes.

Requirements

• Associate's Degree in Nursing or a nursing diploma upon hire.
• California Registered Nurse License upon hire.
• 2 years of acute care experience in a Medical Surgical or Inpatient setting.
• 1 year of experience in care management or utilization review in any setting, OR successful completion of TIPS program or Case Management Orientation Program upon hire.

Qualifications

• Bachelor's Degree in Nursing or higher within 3 years of hire (preferred).
• Certified Case Manager (CCM) or Accredited Case Manager (ACM) (preferred).

Pay

The pay range for this position is $54.61 - $92.23 per hour. These amounts are the base pay range; additional compensation may be available for shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.

Schedule

• Full-time.
• 8-hour, Day shifts.
