RN Manager - Care Management Full-time Day

About the role

We are seeking an experienced RN Manager - Case Management to lead care coordination efforts in an inpatient setting.

Key Responsibilities

• Oversee and facilitate collaborative case management processes, including assessment, planning, intervention, and care coordination to meet patients' healthcare needs.
• Coordinate communication among patients, caregivers, healthcare providers, and community resources to ensure seamless transitions of care.
• Advocate for patients' rights, social determinants of health, and address barriers such as mental health, substance use, and resource limitations.
• Lead and support a team of care managers, promoting best practices and continuous improvement.
• Monitor and evaluate care plans to optimize health outcomes and resource utilization.
• Ensure compliance with healthcare regulations and standards.
• Collaborate with post-acute care providers and community agencies to facilitate patient needs.

Qualifications & Skills

• Bachelor's Degree in Nursing with active California RN license.
• Minimum 5 years of healthcare experience, including at least 3 years in acute care case management.
• At least 1 year of healthcare leadership experience.
• Preferred: Master's Degree in Healthcare or related field, CCM or ACM certification.
• Strong communication, leadership, and organizational skills.
