RN Manager - Care Management Full-time Day

We are seeking an experienced RN Manager - Case Management to lead care coordination efforts in an inpatient setting. Key Responsibilities Oversee and facilitate collaborative case management processes, including assessment, planning, intervention, and care coordination to meet patients' healthcare needs.Coordinate communication among patients, caregivers, healthcare providers, and community resources to ensure seamless transitions of care.Advocate for patients' rights, social determinants of health, and address barriers such as mental health, substance use, and resource limitations.Lead and support a team of care managers, promoting best practices and continuous improvement.Monitor and evaluate care plans to optimize health outcomes and resource utilization.Ensure compliance with healthcare regulations and standards.Collaborate with post-acute care providers and community agencies to facilitate patient needs. Qualifications & Skills Bachelor’s Degree in Nursing with active California RN license.Minimum 5 years of healthcare experience, including at least 3 years in acute care case management.At least 1 year of healthcare leadership experience.Preferred: Master’s Degree in Healthcare or related field, CCM or ACM certification.Strong communication, leadership, and organizational skills.
