About Our Client:The organization operates a busy outpatient neurology practice with multiple locations across California. It focuses on delivering high-quality patient care to complex neurological patients, addressing challenges in managing chronic neurological conditions through specialized care programs. About the Opportunity:The CARE MANAGEMENT role supports care delivery under the supervision of a licensed physician by managing patient panels enrolled in chronic and principal care management programs. This position is essential to coordinating care for patients with complex neurological conditions, ensuring adherence to care plans, and facilitating successful treatment outcomes. Responsibilities:Maintain expert knowledge of care management scope of practice and perform tasks accordinglyUnderstand the organization's care management programs and fulfill required tasksCollaborate with Referrals and Prior Authorizations to complete patient ordersManage an active panel of complex neurological patients including those with neuroimmunological disorders, Alzheimer's, dementia, movement disorders, and infusion therapy Requirements:Minimum one year of care or case management experience preferredMinimum one year outpatient setting experience preferredBilingual English/Spanish preferredCurrent and unrestricted California Licensed Vocational Nurse (LVN) license preferred Pay Range and Compensation Package:The pay range and compensation package for this role will be determined based on the candidate's experience, skills, and other relevant factors.
