Qualifications

• Education: A minimum of an Associate's Degree (Bachelors preferred) in a Human Service field or full certification as an Addictions Counselor
• Experience: Associate's Degree: Five (5) years; Bachelor's Degree: Three (3) years; Master's Degree: Two (2) years
• Certified Addictions Counselor (CAC): Five (5) years

Responsibilities

In this role, you may include:

• Providing case management services, discharge planning and care coordination for individuals transitioning from an ASAM Level Residential Substance Use Disorder Treatment (SUD) Program (3.7, 3.5, 3.3, 3.1)
• Handling a caseload of clients to aid them in achieving adjustments to specific barriers and situations of a difficult or complex nature by connecting them with the appropriate community resources
• Preparing reports, treatment plans, discharge/continuing care plans, and keeping necessary records within client database and electronic health records
• Coordinating the activities of one or more groups of clients
• Generating referrals and aftercare plans
