Qualifications
- Registered Nurse (RN) with behavioral health experience
- Experience assessing and managing patients with behavioral and mental health conditions
Job Summary
The Care Coordinator is a licensed clinical professional who coordinates, facilitates, and supports patient care across the continuum through clinical triage, care coordination, patient education, and proactive patient engagement.
Through telephonic and virtual interactions, the Care Coordinator receives patient concerns, assesses available clinical information, prioritizes patient needs using established protocols, and coordinates appropriate interventions.
Registered Nurses (RNs) may independently perform comprehensive nursing assessments, develop nursing care plans, and exercise clinical judgment within the RN scope of practice.
Licensed Practical Nurses (LPNs) provide nursing care and coordination within the LPN scope of practice, follow established clinical protocols, and escalate complex, unstable, or out-of-protocol situations to the appropriate RN or licensed provider.
