About the role

RiseWell Community Services is looking for a Full-Time Care Coordinator in our State Aid Program. This role offers remote work; however, it would require occasional work in the field (NYC & Long Island Regions) serving participants.

Responsibilities

  • Provide care coordination and health promotion services to individuals throughout Long Island.
  • Provide care coordination to those with severe mental illness and/or 2 or more chronic health conditions to avoid costly inpatient hospitalizations, emergency room visits and unnecessary care.
  • Aid individuals in obtaining Food Stamp and Social Security benefits.
  • Oversee and coordinate access to all services (behavioral, medical health and social service supports) to assist individuals to stay healthy.
  • Complete all required documentation according to standards and no later than 24 hours of service delivery.

Requirements

  • Minimum of a Bachelor’s degree with at least 2 years relevant experience serving children; OR Registered Nurse with 2 years relevant experience serving children; OR Master’s Degree with 1-year relevant experience serving children.
  • Clean, valid NYS Driver’s License.
  • Fingerprinting, criminal record check, and all necessary clearances from the Justice Center.

Qualifications

  • Minimum of a Bachelor’s degree with at least 2 years relevant experience serving children; OR Registered Nurse with 2 years relevant experience serving children; OR Master’s Degree with 1-year relevant experience serving children.

Skills

  • Strong communication skills.
  • Ability to work independently and as part of a team.
  • Knowledge of mental health and chronic care management.
