Care Coordinator

Responsibilities

• Conduct psychosocial and functional assessments
• Develop individualized discharge plans
• Facilitate interdisciplinary communication and care progression
• Address barriers to timely discharge
• Collaborate with care team members, physicians, and patients to support care transitions
• Manage documentation accurately
• Participate in rounds and care conferences
• Ensure resource availability
• Monitor care delays
• Facilitate post-acute referrals

Qualifications & Skills

• Minimum of 1 year nursing experience in acute care
• RN licensure (single or multi-state)
• BLS certification
• Excellent communication, organizational, and problem-solving skills
• Ability to work collaboratively, handle stress, and adapt in a fast-paced environment
• Knowledge of regulations and community resources is essential
