Care Coordinator (RN) - Primary Care Clinic - Full time
Position Overview
The Care Coordinator uses a team-based approach focused on a continuous partnership between the patient, family, caregiver, physician, clinicians, hospital, and community resources to provide coordinated care for complex patients. The Care Coordinator is responsible for direct and indirect patient care across the care continuum, with an emphasis on assisting high-risk patients in the community, including Medicare, Medicaid, VHA, and other payor mix beneficiaries. Activities will promote, maintain, and improve the health of patients by creating and maintaining a centralized, multidisciplinary care plan and providing social support, advocating for individuals, addressing community health needs, and performing community outreach such as health screenings.
Requirements
- At least two years of experience in a clinical setting as a Registered Nurse, Advanced Practice Provider (NP/PA), Clinical Social Worker, or similar.
- Absolutely unrestricted license to practice as a Registered Nurse (RN) in the State of Colorado, or ability to obtain prior to start date.
- Basic Life Support (BLS).
- Bachelor's (BSN) or Master's (MSN) in Nursing preferred.
- Care coordination or case management experience preferred.
- Experience with EPIC preferred.
- Bilingual in Spanish preferred.
