RN Care Coordinator - Full-Time / Day

About the Role

The Care Coordinator RN (CC RN) assesses patients' transitional care needs, coordinates care across the continuum, and collaborates with patients, families, and interdisciplinary teams to ensure effective discharge planning and care progression. Conducts psychosocial, functional, and chronic disease assessments, identifying barriers to discharge and addressing social determinants of health.

Responsibilities

• Assess patient needs and psychosocial risks; collaborate on care and discharge plans.
• Manage all aspects of discharge planning, addressing barriers and ensuring timely transitions.
• Facilitate communication among care team members to promote efficient care progression.
• Document care activities thoroughly and accurately in medical records, tracking avoidable days and trends.
• Participate in professional growth activities, support organizational goals, and mentor students.

Requirements

• Associate’s or Bachelor’s degree in Nursing.
• Active RN license (single or multi-state compact).
• BLS certification.
• Minimum one year of nursing experience in an acute care setting.
• Excellent communication, assessment, organizational, and problem-solving skills.
• Knowledge of healthcare regulations, community resources, and care coordination best practices.
