The Care Coordinator RN (CC RN) is responsible for assessing patient needs, coordinating care across the continuum, and engaging patients and families to ensure care goals are met. Key duties include: conducting psychosocial and functional assessments, planning and managing discharge, facilitating interdisciplinary communication, and addressing barriers to timely care and discharge. Collaborates with physicians, care teams, and community resources to support care progression and resource utilization. Maintains accurate documentation of assessments, care plans, and interactions, ensuring compliance with policies. Supports professional development, mentors colleagues, and participates in departmental initiatives. Qualifications include: a nursing degree (Associate or Bachelor's preferred), RN license (single or multi-state), BLS certification, and at least 1 year of acute care nursing experience. Required skills: excellent communication, organizational, and problem-solving skills; ability to work collaboratively; knowledge of healthcare regulations and community resources; self-motivated and adaptable in a fast-paced environment.
