RN Care Coordinator - Full-Time / Day

About the Role

The Care Coordinator RN (CC RN) is responsible for assessing patients' transitional care needs, coordinating care across the continuum, and engaging with patients and families to ensure care requirements are met.

Responsibilities

• Conduct psychosocial and functional assessments
• Develop discharge plans
• Facilitate interdisciplinary rounds
• Collaborate with physicians, care teams, and community resources to promote efficient care progression and discharge
• Manage discharge planning
• Identify and resolve barriers to care
• Ensure accurate and timely documentation
• Mentor and support department goals as needed

Qualifications

• Minimum of 1 year of acute care nursing experience
• RN licensure (single or multi-state)
• BLS certification
• Strong communication, organizational, and problem-solving skills
• Proficiency with local, state, and federal regulations
• Ability to work collaboratively in a fast-paced environment
