Care Coordinator (RN) - Full-Time Days

About the role

The Care Coordinator RN (CC RN) assesses patients' transitional care needs, coordinates care across the continuum, and engages with patients and families to ensure care plans are met.

Responsibilities

• Perform psychosocial and functional assessments of patients.
• Develop individualized discharge plans.
• Facilitate interdisciplinary communication among care teams.
• Manage post-acute referrals and coordinate with physicians, social workers, and payers.
• Promote timely discharge and efficient resource utilization.
• Identify and resolve barriers to care.
• Document assessments, care progression, and discharge activities accurately and promptly.

Requirements

• Minimum of one year of acute care nursing experience.
• Associate or bachelor’s degree in nursing.
• Current RN licensure and BLS certification.
