About the role

The Care Coordinator is responsible for coordination of activities related to a specific patient population and the administration of direct patient care for a specific unit and shift. The Coordinator functions as a leader of multidisciplinary teams, providing and coordinating care to patients and families.

Responsibilities

• Concurrent utilization and service review
• Consultation
• Patient and family education
• Promotion of continuity of care
• Preceptorship
• Committee work

Requirements

• Current RN licensure or compact licensure recognized by the State of Vermont is required.
• ADN required, BSN preferred.
• Three years of relevant RN experience.
