About the role
Integrated Services for Behavioral Health (ISBH) is a community-minded, forward-thinking behavioral health organization serving Southeastern and Central Ohio. We provide comprehensive behavioral health and other services, collaborating with local partners to promote healthy people and strong communities.
Responsibilities
- Service linkage and care coordination, engaging and working with children, youth, and families with significant behavioral health needs.
- Define cultural factors influencing strengths, functioning, and family interaction styles to ensure ongoing engagement and success in care planning.
- Identify strengths of children, youth, and families for utilization in care coordination engagement and supporting healthy outcomes.
- Coordinate family-based services for children, youth, and families in their home, school, and community.
- Ensure services identified on care plans are the most appropriate, least restrictive, and meet the safety and treatment needs of the child, youth, and family.
- Build positive relationships with children, youth, and families in coordination with child and family teams to support the successful integration of team members and care plans.
- Develop collaborative and creative partnerships with community resources to meet the diverse needs of youth and families.
- Maintain necessary documentation, participate in program evaluation, attend team and program planning meetings, cross-systems training, and acquire knowledge of community resources.
- Remain current with all training requirements, including but not limited to High Fidelity Wraparound, MI, Cultural Humility, etc.
- Perform other duties as assigned.
Requirements
- Experience providing services and/or support to children and families connected to behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral healthcare field.
- Knowledge and experience in Hi-Fidelity Wraparound preferred (Certification provided at time of employment).
- Two years of experience in a coordinated supportive services or care coordination role preferred.
- Experience working with people with autism spectrum disorders and developmental disabilities preferred.
- Experience in one or more of the following areas: family systems, community systems and resources, case management, child and family counseling or therapy, child protection, child development.
- Culturally humble or responsive with training and experience to manage complex cases.
- Qualified to work with children and families experiencing serious emotional disturbance (SED), trauma, co-occurring behavioral health disorders, and engaged with one or more child-serving systems.
- Excellent organizational skills with the ability to stay focused and prioritize multiple tasks.
- Demonstrates a high degree of cultural awareness.
- Experience with multi-need individuals and families.
- Broad knowledge of community service systems.
- Willing to participate in and lead cross-systems care coordination.
- Effective communication through verbal/written expression.
- Ability to operate in an Internet-based, automated office environment.
- Valid Driver's License required.
- Ability to operate, receive, and conduct video calls and/or meetings in an automated office environment.
- Minimum of 100mbps for high-speed internet connection.
Qualifications
- Three years with a high school diploma or equivalent; or
- Two years with an associate degree or bachelor's degree; or
- One year with a master's degree or higher.
