About the role
This position is remote work from home with travel to Nursing facilities required. Hours of operation are M-F from 8:00am to 5:00 pm. Applicants must be in for the Roanoke/ Lynchburg/ Danville area.
Responsibilities
- Performs a variety of casework duties and provides case management services to Members in Nursing Facilities, including patients, families, and designated caregivers.
- Requires travel to assigned facilities.
- Develops, participates, and monitors multidisciplinary collaboration of services to patients where appropriate.
- Affords assistance to adult patients and their families with personal and environmental difficulties associated with medical condition up to and including at time of terminal illnesses.
Requirements
- Bachelor’s Degree (Required) in Health & Human Services or a related field such as Art Therapy, Behavioral Sciences, Child Development, Cognitive Sciences, Community Mental Health Counseling, Counselor Education, Early Childhood Development, Educational Psychology, Gerontology, Healthcare Administration, Human Development, Human Services, Marriage and Family Therapy, Music Therapy, Nursing, Pharmacy, Psychology, Rehabilitation Counseling, Social Work, Sociology, Special Education, Speech Therapy, Therapeutic Recreation, Vocational Rehabilitation.
- One year of Long-Term Care and Health Plans experience (Required).
- Nursing Home Experience (Preferred).
- Care Management/Care Coordination experience (Preferred).
- Experience with Community Health Resources (Preferred).
- Experience initiating care plans and both subjectively and objectively conducting assessments (Preferred).
