Care Coordinator - CSP
About the role
Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester County. We are seeking a full-time Care Coordinator to join our SRH administration site located in Peekskill, New York.
Responsibilities
- Coordinates with patient and health providers to facilitate timely follow-up services.
- Provide in-reach to patients within existing client base to help them understand the importance of preventive services; help patients understand recommended follow-up of abnormal screening exams & treatment referrals.
- Works with CSP participating providers within the service area and other community resources to become knowledgeable about available services that may be helpful to patients.
- Aid patients in overcoming logistical barriers to scheduled diagnosis and treatment appointments.
- Contact patients “at risk” for missing appointments to improve compliance with recommended follow-up.
- Collect information required to complete CSP database reporting forms & provide timely / accurate entry on the Catalyst data system.
- Coach patients to become advocates for their own care.
- Builds awareness of care/data coordination and CSP services among the healthcare team by building professional relationships with team members, providing information about CSP and maintaining communication to identify patients who are “at risk” with barriers to care.
- Communicate with providers about unique patient needs (i.e. cultural).
- Facilitate access to available resources for obtaining insurance coverage and/or sliding fee benefits for clients not CSP eligible.
- Assist to ensure that appropriate information is available in the patient’s medical records during scheduled appointments.
- Demonstrates the ability to collaborate effectively and professionally with external partners, including but not limited to Open Door/HITCH and affiliated community organizations and maintains clear, respectful communication, fosters strong working relationships and ensures coordination of services aligns with organizational standards and patient care objectives.
Requirements
- High School diploma/GED, Bachelor's Degree preferred.
- Driver's license required.

$24.00 - $27.35 per hour
