About the role
The Care Coordinator – Clinics supports the clinical area by coordinating care to ensure continuity while providing services. They collaborate with clinical teams, linking consumers to community-based resources and supports. This position works closely with clinical treatment teams, identifying barriers to care and assisting with implementation of supportive treatment plans.
Responsibilities
- Collaborate with clinical teams to coordinate care and ensure continuity of services

- Work closely with clinical treatment teams to provide support
- Support the clinical area by actively assisting with coordination of care
Requirements
- Bachelor’s degree in human services related field (e.g., Social Work, Education, Psychology, Sociology)
- At least one year of satisfactory work experience working with individuals with intellectual and/or developmental disabilities, mental illness or substance use disorders
- Thorough working knowledge of professional ethics and standards of practice
- Computer skills including word processing, spreadsheets, email, database management, and data entry skills
- Demonstrated experience communicating clearly and concisely in stressful situations
- Excellent communication and customer service skills
- Ability to work effectively in a multicultural environment and bridge language/cultural barriers to Network180 services within diverse communities
Qualifications
- PREFERRED QUALIFICATIONS: Three years post-undergraduate experience working with individuals with mental illness and co-occurring disorders. Spanish/English bilingual. Lived experiences with mental illness/developmental disabilities/substance use disorders valued.

Pay
$21.99 - $29.15 per hour, final job offer based on internal equity, skills, education, experience, and credentials.
Schedule
Full-time, Non-Exempt. Works 40 hours a week, Monday through Friday, 8:00 am to 5:00 pm. In-person at 790 Fuller AVE NE, Grand Rapids, Michigan 49503.
