Care Coordinator/Case Management

Travel assignment starting 9/15/2026 for 17 weeks, 40 hours per week (8-hour day shifts, every other weekend).

Coordinate patient care across providers, oversee utilization review to ensure appropriate and cost-effective services, and facilitate timely, safe discharge planning by arranging post-acute resources and support.

Requirements

• Active California RN license
• BLS certification
• 3 years of hospital experience
• Acute-level care coordination experience
• Experience with discharge planning and utilization review
• Prior CA Dignity/Commonspirit acute care experience in specialty (preferred)
• Minimal time-off requests

Pay

$1,800.00 - $2,500.00 per week
