About the Role
New York Psychotherapy and Counseling Center (NYPCC) is seeking an energetic, bright, and self-motivated Care Coordinator to join our team. This is a full-time position based out of our Child and Family Health Center at 91-09 Roosevelt Ave, Jackson Heights, NY 11372. Gateway to Wellness is a Health Home Care Management initiative that supplements and enhances the behavioral health services we offer throughout the NYC area.
Responsibilities
- Manage a caseload of 85+ Health Home Care clients
- Assist in developing Comprehensive Care Plans
- Address various service needs including housing, benefits, medical care, transportation, education, employment, crisis intervention, and other supportive services to enhance client quality of life
- Work as a member of the Care Team including Supervisors, Clinicians, Verbal Psychotherapists, and Psychiatrists
- Execute advocacy, assessment, service planning, creating linkages/referrals, and ongoing documentation and monitoring of Electronic Health Records
- Contact individuals diagnosed with mental illness, substance abuse disorders, and chronic medical conditions that significantly impact functioning on a monthly basis in person and by phone
Schedule
Hybrid schedule Monday–Friday, with mandatory onsite presence Monday, Tuesday, and Thursday.
Qualifications
- Must be bilingual (English/Spanish)
- Bachelor's Degree required
- Experience with RMA required
- Experience with HARP clients preferred
- Knowledge of various community resources and services to assist with service delivery and linking members to needed services based on a client-centered service plan
- Excellent verbal and written communication skills to provide linguistically appropriate services to assigned caseload
- Ability to communicate with other professionals, network providers, and managed care organizations regarding client statuses, level of functioning, and needs for additional services
