Sentara Health is hiring a Full Time Care Coordinator for the Hampton Road area!

Hours

Full Time (40 hrs/wk)

Day (8am-5pm)

Education

• Must possess a Bachelor's degree in Health & Human Services or one of the following related Fields:

• Psychology
• Rehabilitation Counseling

• Therapeutic Recreation
• Vocational Rehabilitation

Certification/Licensure

No specific certification or licensure requirements

Experience

• 1 year of social work, care coordination, case management, behavioral health, healthcare, or community-based services experience (Required)
• Experience conducting psychosocial, or needs-based assessments and developing individualized care plans (Preferred)
• Experience advocating for patients, clients, or vulnerable populations and assisting with care transitions and service navigation (Preferred)
