About the role
Sentara Health is hiring a Full Time Care Coordinator for the Roanoke, VA area. The position involves travel for in-person face-to-face assessments in Roanoke, VA, with flexibility to work remotely during days without assessments scheduled.
Responsibilities
- Support individuals with Intellectual and Developmental Disabilities (ID/DD) and waiver services
- Coordinate treatment or recovery support services for individuals affected by substance use disorders
- Advocate for patients, clients, or vulnerable populations and assist with care transitions and service navigation
Requirements
- Must possess a Bachelor's degree in Health & Human Services or a related field such as Art Therapy, Behavioral Sciences, Child Development, Cognitive Sciences, Community Mental Health Counseling, etc.
- No specific certification or licensure required
Qualifications
- At least 1 year of social work, care coordination, case management, behavioral health, healthcare, or community-based services experience
- Experience supporting individuals with Intellectual and Developmental Disabilities (ID/DD)
- Experience working with individuals affected by substance use disorders and coordinating treatment or recovery support services
- Experience advocating for patients, clients, or vulnerable populations and assisting with care transitions and service navigation
Skills
- Strong interpersonal skills
- Excellent communication and problem-solving abilities
- Ability to manage multiple tasks and prioritize effectively
