Behavioral Health Care Coordinator

Responsibilities

  • Provide Care Coordination and support for youth and families receiving High Fidelity Wraparound (Tier 1) or Intensive Care Coordination (Tier 2) services.
  • Work collaboratively with clients, family members, and multidisciplinary providers to develop, implement, and monitor individualized plans.
  • Conduct in-person, community-based child and family team meetings in accordance with program and model requirements.
  • Provide assertive outreach, advocacy, support, and follow-up to assigned clients; document all outreach attempts and ensure services outlined in the personal growth plan are delivered at the identified frequency.
  • Deliver case management services to youth enrolled in the Family Support Program (FSP) and/or Specialized Family Support Program (SFSP).
  • Assist clients and families in building and sustaining natural support teams, integrating community supports and resources across identified life domains.
  • Incorporate natural and community-based supports into both the personal growth planning process and ongoing service delivery.
  • Present relevant clinical and service information to ensure quality of care, obtain feedback, and guide service planning and adjustments.
  • Identify, document, and report Critical Incidents in accordance with Centerstone policy and procedures.
  • Support crisis planning and prevention efforts, including evaluation of safety and prevention plans.
  • Provide community education and outreach activities as assigned.

Qualifications

  • Education Level: Bachelor’s Degree in a human services-related field, OR Associate’s Degree in human services plus at least three years of hands-on experience in mental health, OR High School diploma or equivalent with a minimum of five years of supervised experience in behavioral health (subject to approval).
  • Years Of Experience: Prior experience required as detailed above.
  • Certification/Licensure: N/A
