Responsibilities

  • Engaging and collaborating with facility staff, including Executive Directors, Directors of Nursing, case managers, social workers, staff developers, admissions personnel, surveyors, and physicians.
  • Facilitating the coordination of a variety of services, including Psychiatry, Therapy, and Neuropsychology (where available), to meet the needs of patients in the facilities.
  • Aiding facilities in processing referrals and identifying residents who could benefit from psychiatric and therapy services.
  • Coordinating the completion of consents and providing assistance when needed.
  • Educating facility staff on Behavioral Health Solutions processes, addressing questions, and resolving concerns.
  • Attending behavior management meetings with facility staff and coordinating MD attendance for facility meetings such as GDR and QAPI.
  • Ensuring compliance with PASRR II requirements by working closely with both BHS and the facility.
  • Scheduling in-services and educational sessions for facility staff.
  • Facilitating Telehealth visits with clinicians as needed to enhance patient care.
  • Collaborating with the Business Development team to support the facility onboarding process.
  • Performing additional tasks and assignments as specified by your supervisor.

Requirements

  • An individual must be able to perform each essential duty satisfactorily.
  • A minimum of a High School Diploma or GED, with some college education focused on behavioral health or psychology preferred.
  • Experience: 3-5 years of experience working in skilled nursing facilities as a Licensed Social Worker, Support Staff, or Clinical Care Coordinator.
  • Proficiency in using a PC, Windows environment, Internet, and MS Office Applications such as Outlook, Word, and Excel.
  • Proficiency in using EHR software systems (Athena) is preferred.

Pay

$55,000 - $62,500 annually
