About the Role

1.0 FTE (40 hours per week), non-exempt position.

Pay

$24.09–$29.51/hour, depending on experience. Additional pay for bilingual skills, based on hours worked.

Responsibilities

• Complete CANS Assessment and screen individuals to determine eligibility for the WISe program using state-mandated criteria.
• Facilitate Child and Family Team (CFT) meetings and guide the wraparound process through all phases, ensuring all CFT members receive appropriate documents.
• Utilize consensus-building techniques to meet the needs of youth and families, encouraging CFT members to identify priority concerns and minimize potential conflicts.
• Recruit and identify natural supports to join the CFT to assist in the youth/family’s stabilization and community functioning.
• Serve as the central point of communication for CFT members and coordinate team-based planning.
• Provide case management services, identifying needs such as transportation, food, medical services, and housing, and teach families how to access community resources.
• Document all referrals made to other resources.
• Provide supportive counseling using behavioral strategies for individuals and families.
• Develop and monitor individual care plans, adapting strengths-based goals using CANS Assessment scores to track progress and achieve stability.
• Establish and maintain rapport with clients to encourage participation and retention in the program.
• Facilitate crisis support meetings and develop collaborative crisis support plans for individuals/families.
• Monitor and adapt plans to stabilize and support youth during crises.
• Provide 24-hour crisis support to clients enrolled in WISe via phone or in-person response (home, hospital, or community settings).
• Complete Full CANS Assessments as required to identify strengths, needs, and monitor progress.
• Meet or exceed productivity standards for the position.
• Document activities, concerns, and behavioral issues in participant records, ensuring compliance with funder and outside organization requirements.
• Participate in community meetings to collaborate with professionals (e.g., schools, courts, DSHS, correctional facilities, healthcare providers) in treating individuals.
• Prepare reports and documentation for outside organizations, attend meetings, and represent the agency as requested.
• Convey treatment recommendations and collaborate with mental health professionals and community partners to address mental health needs in a strengths-based manner.
• Perform duties in alignment with The Joint Commission (TJC), Health Resources and Services Administration (HRSA), and YVFWC requirements.
• Perform other duties as assigned.

Requirements

• Bachelor’s Degree in Social Work, Psychology, Education, Counseling, or a related field.
• One year of experience working with individuals or families in a healthcare, social service, therapeutic, or educational environment.
• Application for registration as an Agency Affiliated Counselor in Washington must be submitted within one month of hire.
• Valid Driver’s License and proof of automobile liability insurance coverage.
• BLS certification within 45 days of employment.
• Ability to work flexible hours, including evenings and weekends, and participate in on-call rotation.
• Reliable transportation for travel to meetings and visits at other YVFWC sites; occasional transportation of clients may be required.

Preferred Qualifications

• Experience working with children and families.
• Spanish-speaking skills at a level 9 (eligible for Bilingual Differential Compensation upon achieving an ALTA score of 9).
• Knowledge of Trauma Informed Care, State mental health and special education requirements, and Mental Health First Aid.
• Knowledge of Children’s Behavioral Health/WISe principles and cultural differences impacting client support.
• Ability to assess learning capabilities of individuals and family members.
• Strong independent judgment, decision-making, and conflict resolution skills.
• Proficiency in Microsoft Outlook, Word, and Excel.

Work Environment

This position may involve exposure to secondary trauma while providing direct services to community members, including those with behavioral health challenges, trauma exposure, gang involvement, substance abuse concerns, mental health diagnoses, homelessness, or tendencies toward violent behavior.
