Care Coordinator

About the role

We're searching for a Care Coordinator who'll coordinate comprehensive, patient-centered care across the continuum. You'll partner with physicians and interdisciplinary teams to assess needs, plan and manage services, and remove barriers to care. It's a role focused on improving patient outcomes, service, satisfaction, and cost effectiveness while advocating for patients and their families at every step.

Responsibilities

• Performs comprehensive biophysical, psychosocial, environmental, discharge, and financial assessments to develop individualized, evidence-based care plans
• Coordinates care across the continuum and provides ongoing case management, partnering with physicians to allocate resources according to patient risk
• Documents and facilitates interdisciplinary plans, ensuring continuity across all patient touch points
• Proactively identifies patients needing coordination and works to identify and resolve barriers to care
• Monitors patient progress and revises care plans as needs and circumstances change
• Communicates and collaborates with multidisciplinary teams through team rounds and patient-centered problem-solving
• Reviews clinical documentation, level of care, patient placement, resource utilization, and transitions
• Advocates for patients and families, supporting self-care, rights, and safe transitions between settings
• Conducts visits and community education and outreach, facilitating access to providers and community resources
• Tracks quality, safety, outcomes, utilization, and cost, and participates in performance improvement committees
• Maintains confidentiality and supports teamwork, mentoring, and ongoing professional development

Qualifications

• Bachelor's degree in nursing; or associate degree or technical diploma in nursing with current enrollment or acceptance in a BSN program and a signed agreement; or associate degree in a related field accepted by the Texas Board of Nursing for purposes of obtaining and maintaining RN licensure; or current enrollment in a nursing bridge program toward an MSN with verified BSN equivalency or a signed agreement
• Current RN license from the Texas Board of Nursing or a Nursing Licensure Compact state
• Current BLS certification from the American Heart Association
• Three years of nursing experience in an acute care setting

Skills

• PEARS Certification From The American Heart Association (preferred)
• CCM certification from the Commission for Case Manager Certification (preferred)
• ACM certification from the American Case Management Association (preferred)
• CCCTM certification from the Medical-Surgical Nursing Certification Board (preferred)
