About the role

Sweetser's Certified Community Behavioral Health Clinic (CCBHC) in Sanford is searching for a Care Coordinator. Care coordinators provide support to individuals accessing the Community Behavioral Health Clinic through supporting intake/access to care and ongoing monitoring of care and needs. Care coordinators manage ongoing coordination of care for a caseload of individuals, including facilitating collaboration among providers and linkage to needed services.

Responsibilities

  • Coordinate care for individuals in the Community Behavioral Health Clinic (CCBHC).
  • Facilitate ongoing collaboration and communication between providers.
  • Support intake/access processes by completing risk screening and behavioral health/SUD screenings.
  • Link individuals with needed resources and services, ensuring services meet their needs.
  • Complete timely and accurate documentation of all contacts and activities using an Electronic Health Record.
  • Complete comprehensive surveys with individuals at intake, discharge, and every six months while in care.
  • Track and administer surveys, including data entry.
  • Conduct primary care screening and monitoring processes and procedures.
  • Coordinate vaccinations for Hepatitis A and B series for clients, as indicated by screening procedures.
  • Support individuals in completing applications for applicable benefits, including Mainecare, non-emergency transportation resources, and other social services.
  • Utilize supervisory supports appropriately and participate in all team/program meetings.
  • Other duties as assigned.

Requirements

  • Bachelor’s degree required.
  • 1-2 years of mental health experience preferred.
  • Intermediate computer skills.
  • Demonstrates professionalism and collaboration with others.
  • Ability to receive and implement professional feedback from supervisors and colleagues.
  • Recognize signs of increasing clinical or situational risk and utilize de-escalation and safety techniques effectively.
  • Manage multi-task situations, conflicting demands, and stressful situations while meeting critical deadlines.
  • Maintain current knowledge in present practice area through required training and clinical supervision.
  • Strong listening skills, flexible collaboration, and a consistent problem-solving focus.
  • Communicate ideas and opinions clearly and constructively, both verbally and in writing.
