About the role

Partner with families, service providers, and community resources to support children aged 0–3 with developmental delays or disabilities. Conduct assessments, develop and implement family-centered service plans, coordinate services, and facilitate team meetings. Provide education, advocacy, and maintain accurate documentation in compliance with regulations. Collaborate internally and externally, managing a flexible schedule to meet client needs, including occasional evening or weekend hours. Ensure timely service delivery and meet productivity expectations.

Qualifications

  • Bachelor’s degree in Social Work, Child Development, or related field
  • Preferred experience working with families with special needs
  • Valid driver’s license, reliable transportation, and strong organizational, communication, and documentation skills
  • Ability to work in various settings and respond appropriately to crises

Work Environment

Community-based role involving daily travel, physical activity, and exposure to diverse environments in homes and communities. Travel may include occasional overnight stays.
