About the Opportunity

The Care Coordinator serves as the primary contact for families, guiding them through the complexities of caregiving. This role is responsible for managing care logistics, building strong relationships, and prioritizing individual care needs. The coordinator develops comprehensive care plans and oversees ongoing family needs, ensuring seamless service and relieving families from administrative burdens, thereby enabling them to focus on their loved ones.

Responsibilities

  • Act as the main point of contact for families managing complex care.
  • Develop and implement comprehensive care plans tailored to family needs.
  • Coordinate medical appointments, research providers, and track medications.
  • Manage medical bills, insurance matters, and facility vetting.
  • Collaborate with Care Advisers to provide seamless and high-quality service.
  • Identify and suggest process improvements to enhance care delivery.
  • Maintain consistent communication and accountability with families.
  • Mentor team members by sharing insights and best practices.
  • Represent the organization at external events to promote its services.
  • Participate in cross-functional initiatives to improve workflows.

Requirements

  • Bachelor’s degree in Social Work, Public Health, Healthcare Administration, or related fields; advanced degrees preferred.
  • 4 to 6+ years of experience in healthcare, specifically case management, crisis intervention, or coordination.
  • Strong knowledge of the healthcare sector and empathetic, clear communication skills.
  • Experience in developing and facilitating care plans.
  • Ability to manage up to 40 care projects efficiently with compassion.
  • Adaptability and receptiveness to constructive feedback.
  • Preferred: experience with complex healthcare situations, multilingual abilities, and proficiency with macOS, Zoom, GSuite, Slack, and 1Password.

Pay

Salary Range: $24.00 to $27.00 per hour, including training.
